What's the condition of your fork's bearing race? Have you "installed" a new fork?

I don't understand. What cap?

The only problem I've had with a wobbly/rattling fork was forgetting to install on my new fork the bearing race from my old fork. A lot of the new forks have said race machined right into the crown. And what I've just said implies the use of an integrated headset. I haven't messed with a "standard" headset since the '80s.

Again, I'm not sure what you mean in your above post...

Very old... but still not the "oldest."

Based on the fact that tightening the compression bolt makes it hard to turn, yet it still rattles leads me to believe it's a bearing race issue. Again, have you installed a new fork? Do you have an integrated headset? If the answer to both of those questions is "yes," did you remove the bearing race from the old fork and put it on the new fork before you installed it?

If this rattling just came outta nowhere or if you installed a new fork with a "built in" bearing race or you're using a standard headset, I'm out of "solutions."

But if he were "missing" spacers how could he tighten the compression cap so much that the steering gets "stiff?" If he doesn't have "enough" spacers - regardless if they're above or below the stem - his compression cap will just torque down on the top of the fork tube. What am I missing!?!

It's 6:00 am here. I must be going. Sorry I couldn't help, glubbers...

Agreed, bearing race. I just can't figure why, if he used his old fork, the race isn't still there. I mean, surely, he wouldn't have taken it off.
