Okay so when riding my bike i feet something off so when i checked my back wheel could slightly move side to side so i took it off and realized the axle was a little bit loose so i tightened the nuts for the hub and that still didn't work
How do i fix this?

It could be blown out bearings.

If the bearings are sealed, tightening the cones won't stop it from wobbling. You probably need new bearings

Worn/bent shell or axle?

Also, when you tighten your bolts when putting the wheel on, ensure that the cone nuts aren't moving as you tighten the wheel into you dropouts.
