I can get into a smith about half the time, but whenever i try to hop out i end up doing a toothpick to almost faceplant. What do i fix?

Keep most of your body weight leaning back
When learning smiths the hardest thing to learn is to hop out of them but once you get it down it'll be easy

Maybe I should put my pegs back on, grinding seemed pretty easy for me. Anyway, keep your weight over your back wheel and when you hop off push your back wheel down like if you were doing a drop

And for me Blaaaa It was getting in because my front peg would grip every time and throw me over, I decided to just go faster and lean back to get it moving and it worked.
Later i found that they work better on anything but wood.
Also you can treat it like a manual out, so whilst you are grinding have your body centered above your rear tire and when getting out sorta treat it like you are at a pump track and really trying to keep your front wheel above the next lip.
Or as if you are doing a manual either way it might help

Yep, nearly faceplanted today, ate a tiny bit of shit OTB. Didn't even use wax either which made it twice as bad on rough concrete.
