How do they "adjust" your chain and how does that help?

I'm pretty sure that when you tighten the bolts, the bolts push against the hub's axle, and it tightens your chain. No sure how you can put them on, but i understand the integrated ones that comes on bikes.

In intergrated chain tensioners, a screw in built goes through the dropouts. As you put your wheel in the dropouts, you screw the bolt so it moves farther out of the dropouts. It pushes against the axle and prevents it from moving, therefore keeping your chain tight.
