i believe that i have bent my rim. every time i spin my rear wheel around it sometimes hits the frame an sometimes doesnt in one spin. i lined up my wheel aswell so the bolts were in line and it was still bent. Also it comes loose occasionally and my tyre starts rubbing against the frame.
So is it a bent rim or is something else happened.

Look up how to true your wheel and go from there

True your wheel, and learn how to correctly tighten your bike centered so it doesn't rub.
